Important Information About Procedures for Opening a New Account
To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account. What this means for you: When you open an account, we will ask you for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents.

MAIL-IN ACCOUNT APPLICATION SPECIAL PROCEDURES

Prior to mailing in your account application, new account customers must have their signature card acknowledged by a Notary Public.
In addition, along with your application you must send a photocopy of one primary and one secondary ID as described below:

Acceptable forms of primary ID*:
- State Driver’s License or ID card
- Commercial Driver’s License
- Foreign Passport
- US Passport or Passport Card
- Military or Dep. of Defense ID
- Federal Government ID
- Alien Registration card (aka Green Card)
- Non-immigrant Visa & Border Crossing Card
- Permanent Resident Card
- INS Certificate
- Native American Tribal ID Card
*State/Country of issuance, number, date-issued, and expiration date must all be listed on signature card.

- Any Primary ID
- US VoterRegistration Card
- Social Security Card (original)
- Major Credit Card (unexpired)
- Bank Debit Card (unexpired)
- US State or Local Government ID
- Current Automobile Registration
- Current Property Tax bill
- Current Student ID
- Current Insurance Card
- Current Firearm License
Addresses referenced on bills or membership cards must match address of account.

Please follow these steps:
- Click on the link to open the appropriate Application Form.
- Complete the form online; or
- You can download or save the application form Acrobat Reader file, and complete the form off-line; or
- You can print a blank copy of the form and complete it later.
- When completing your form from your PC, whether online or off-line, you can delete all of the data you have input at any time by pressing the Reset Form button.
- A "Print Form" button is located at the bottom of application form. Print a copy the completed form.
- Sign and date your completed Application form.
- Send your completed and signed form to us (along with other requested items).
- Mail your completed Application to:

After we have received your completed Application, we will review your request and get back to you within one business day.

No Personal Information Needs to Go Over The Internet
Many consumers are worried that personal financial information they may submit over the Internet could be intercepted by hackers and used for Identity Theft and fraud. You can avoid that possibility by using our convenient Adobe Acrobat Reader® online application forms.
You can complete the Acrobat Reader form online, print the completed form, then sign your form in the presence of a Notary Public and mail the form to us. None of your personal financial information will be transmitted over the Internet.
If you prefer, instead of completing your form online you can either download the application form file and complete it off-line, or print a blank copy of the application form and complete it at your convenience.
